Personalized Support

The Patient Navigation Center is the world’s first personalized support center for people impacted by psoriatic disease. Whether you’ve been newly diagnosed or have lived with your condition for years, patient navigators will work with you one-on-one to get the best care, treatment and support needed to manage your disease. They help you to expect more from and feel better about your health care, your medicines and your future with psoriatic disease. Whether you have one question or a thousand, the Patient Navigation Center is there for you short-term and long-term.

Team of Trained Professionals

Staffed by a team of specially-trained patient navigators, the Patient Navigation Center provides free guidance to all people affected by psoriasis and psoriatic arthritis - patients, families, caregivers, health care providers. They are there to answer specific disease-related questions, offer personalized guidance on access-to-care issues including social security disability, and provide ongoing support to help those impacted.

Patient navigators can even help you achieve your 2017 psoriatic resolutions by helping to set measurable goals, create a custom action plan, and track your progress. From adopting healthier eating habits to lowering monthly prescription costs, the Patient Navigation Center is your key to take control of psoriatic disease in 2017.

It’s important to note that patient navigators do not provide any diagnostic or medical services. They are not a substitute for the consultation of health care professionals, but instead are intended to be used in conjunction with the care provided by your doctors.
